On the first visit, we will go over your health history, perform a biostructural examination, evaluate your goals for care, and make any necessary recommendations for you moving forward.

After scheduling your appointment, an online Health Intake Form will be emailed to you to be fill out ahead of time. This allows us to be  efficient with our time together. A paper copy may be filled out in-office if needed.
